The mainstream media actually gave quite a bit of attention to PEPFAR during President Bush's two terms. I think the reason more Americans aren't aware of the program is that, by and large, as a people we aren't interested about what happens in foreign countries.

Oh sure, royal weddings or natural disasters will hold our attention for awhile. The same is true if Americans are being killed overseas. But those are exceptions to the rule and even those stories have a limited shelf life.
