If it's overt fraud like faking a signature, generally you are correct. If it's stuff like 'filling in the blanks' on routine stuff or documenting in a way to maximize reimbursement rather than to accurately reflect services actually rendered...let's just put it this way: there ain't half enough jails and it's very hard to differentiate incompetence/ignorance of the rules from plain dishonesty...
