Insertions in the Hail Mary?

I've read and heard about the practice of inserting a phrase into the Hail Mary when praying the rosary or the Franciscan crown, such as "... and blessed is the fruit of thy womb, Jesus, whom thou didst joyfully conceive" but I can't find anything that clearly outlines the practice or lists the words for all the mysteries. Has anyone heard of this? Does anyone know how it works?

When I use that sort of meditation, I usually make it up as I go along. :o
