Even then, only actions can be sinful. Being excessively intoxicated and risking harm to oneself or others is sinful, not drinking alcohol. Christ’s very blood in the sacrament has an alcohol content of 22%!
Injecting herione is not necessarily sinful; violating society’s determination that the substance is too dangerous for recreational use is sinful. Taking it as a prescription for pain, as available in some countries, is not sinful if properly managed.
Even taking birth control pills is not sinful; the intent to contraceptive is what is sinful. Taking the same medicine to treat a serious condition can be justified.
