Why would prayers be offered "through the Immaculate Heart of Mary"?

In the prayer, “O Jesus, through the Immaculate Heart of Mary, I offer you all my prayers, works, and sufferings of this day in atonement for my own sins and those of the world, and for the salvation of souls,” what does it mean to offer “through the Immaculate Heart of Mary”?

It is a means of asking Mary’s intercession for our prayer intentions and also recalls that just as Jesus came to us through Mary, so we may approach him through Mary.
